1. Undergo with Pre-Membership Orientation. (The information herein will serve as the internet educator of our Cooperative. Since the Primer is downloadable from our website, this will serve as the initial Pre-Membership Orientation of the aspiring members. If ever the applicant has still some inquiries, the applicant may send it thru Enquiry section and upon satisfying all the applicant’s questions, it will be in accordance to the applicant’s own discretion whether to join or not.

2. Secure an Application Form in our Olongapo City Office or any of our authorized representatives (please refer to our contact details) or download the P14 MPD Coop Membership Application Form.  Please complete the Membership Application Form and submit together with the Waiver for Online Membership, as the case may be.

3. Submit 2 copies of 1 x 1 picture (for application form, and passbook), cedula, Barangay Clearance and photocopy of 2 valid Identification Cards (Driver’s License, Passport, SSS/GSIS, TIN, PRC ID or Company ID).

4. Processing of Application Forms will be done after receiving your deposit payment provided for your membership fee and Share Contribution to our depository bank, as indicated below:
